1. New Patient Intake Form
2. Admission Consent and Authorization
At your first visit with us you will have to complete initial new patient forms and insurance paperwork (required by some but not all insurance companies). You will receive an initial examination with Dr. Fortin which includes a history of your health and an examination that includes orthopedic tests, neurological tests and a chiropractic evaluation to make sure you are a candidate for chiropractic treatments. Along with the examination, you may also be referred out for an X-ray or other imaging if needed for your condition. In some instances, we may even refer you to another health professional if needed. After the initial history and examination are performed, you will also be adjusted that same day. If imaging is needed before an adjustment, then therapy will be given to you to help with your pain.
The first time you see Dr. Fortin, allow at least one hour for this initial appointment. We also ask that you come 10 minutes earlier to your scheduled appointment to allow time to fill out the necessary paperwork. This will also allow Dr. Fortin to have the full hour with you which gives you sufficient time to get all of your questions answered. Dr. Fortin will also explain to you about chiropractic and how it can help your condition at your first visit. Before you leave, instructions will be given to you to help with your pain at home which will help you get better faster between each visit.
If other visits are needed, then the subsequent visits will be about 20 minutes in length unless another condition is presented at your visit, which may require further evaluation.
